He picks you up right outside your house in the morning, so it is impossible to miss him. For some reason the date scene doesn't seem to trigger for you. Maybe advance some other scenes and then do the onsen scene again or something.

When you go in the hotel bar area, you first talk and shag with the naked bartender (the one you're supposed to be dating later on), he will then leave the building. Then there's the two other guys who are alone and the other guy who's talking to the other woman. It is possible to break these scenes by doing the two guys and then talking to one of them for a second time, making the bartender return.
What you're supposed to be doing is: do the scenes with the two guys after the bartender leaves. Then talk to both the woman and the third guy a couple times, which will trigger another scene. After this event you should be able to go back and trigger the date.
If this doesn't work, you can check the CG room for other missing scenes, or take a look at the wounded farmer. If there's two farmers at the animal pen, then you should talk to the wounded version, triggering a bugged version of the doctor scene. This will unlock the missing doctor scene in the CG room.

I just noticed that just like with the farmer who will make you get soft locked if you don't talk to him for the 2nd time and get the panties dialog, you can also get soft locked in Roman Lake area(area where brothel is).
After you see the 2nd scene with this guy on the first picture, do not go to sleep or you will get soft locked and won't be able to progress. Instead go talk to the guy on the 2nd picture. Doing both those things will turn on switch 439 and 440 which will trigger a scene on the stairs when you try to leave the beach.
In case you get the 2nd scene from the guy on the first picture and go to sleep, switch 439 will get turned off and you will never be able to turn it on again which means you won't get the scene on the stairs and other switches and variables won't change that trigger next scenes and events making it impossible to trigger other scenes.
Again this isn't a bug since he intentionally made it to work that way.

It looks to me like you skipped one of the doctor's scenes. I would assume that you can't get it back as you now only see the latest one. Will check later if I have that scene and what it contains.

Go back to the animals and talk to the wounded farmer (there will be two clones). This will give you a bugged scene and you can progress the story that way (date with the bartender, new town, ...)

Can anyone tell me how to drop that stat that gives "game over" after sex? I think i saw some peoples call it pregnancy stat and saying you can buy something to drop that but I am in starting town/village and there is nothing to buy there ... Is there something i can do? I can't progress the story without dropping that stat.

That's the first thing i tried, nothing happened, i am still at 67 and wont go down ...

Yeah... Learned that the hard way in that bar/spa. Had to load save that was 1h behind! Now every save has it's own spot
