I found that my old save files worked for this version. However, it seems like the debug doll thing in the start room (the one that can give a good gun) is no longer there. Or is it just my save...?
Also note that the game over scene is different if you die to two mad scientists as opposed to one.

The whole 'have to kick away the flying worms to get a clear shot' thing was a nice touch. Wish other action games involving tentacles would add in such mechanics.

Figured I'd revive this topic, as the game updated on the third.
Can't figure out what's new though. Doesn't seem to be any new enemies yet, and the fly's are still bugged (I checked after importing my save)

He added the enemies into the mission two of the extras screen. They perform grabs and rapes in there, but the AI heroine always escapes the ones that can be escaped from. He also added in a few hallways and empty rooms past the weird lab area where you go and try to save that girl who is being raped by those tentacles. There is a desk and another door at the final room, but nothing beyond that it seems. He still hasn't finished the fly and it also shows it as unavailable in the mission 2/debug map.

Bumping this thread: this is a great little game, "2D with depth" action games are unusual, let alone with hentai.
Unfortunately I can't progress with this because I think the girl runs out to the beach, then going back is just pummelled by tentacle dogs. Not just this, the backgrounds don't render. I know why: the filenames are messed up, but I've tried extracting them from the ZIP using 7-Zip AND WinRAR, and using AppLocale. No difference at all, meaning I have no idea where to run to return to the "building".

After you talk to the guy in the chair at B building, you need to find a handgun in one of the lockers in the lower floors of that building. Once you retrieve that, head back up to him so he will teach you how to use it. From there, a set of double doors in the building can be entered that contains green slime.
Get past them and the next room is an elevator that you can go down. That level, after you find another key in one of the rooms down there, spawns the scientists.

New update out.
Looked like there was some bugfixes to certain enemies, possibly only related to the sandbox under Extra.
A new enemy type, those little jellyfish looking things that ran away from the body of the big guy, with a pretty gruesome defeat if I am interpreting the dialogue correctly and a poison status effect on hit.
The gun now has the aim wander depending on how fatigued you are, and the lab has some more things added to it, got to a pretty difficult encounter a few rooms after the place you find the girl on the video camera, so I am not sure if there's more after that. I'm not even sure the encounter is survivable.
Edit: Okay, it is survivable, and there's some new locations added outside, beyond the gate that used to be blocked off between the two first buildings, but that's the end of this version.

Double edit: Holy cow. The battle with the billion purple worms is driving me bonkers.
Triple edit: Would you perhaps provide a save file past the billion worms? The fact that getting tackled once is a 90% chance of getting a game over is making this far too frustrating.
How does one go outside the gate between the buildings? I've collected the gun, had the guy teach me to use it, and killed the elevator monster, but inspecting the game gives me a dialogue box and nothing else happens.

I'm not entirely sure which one of these is most directly after that fight, probably the next to last one, but you aren't really missing much, you only pick up a key after that fight.
Oh, and my tip for anyone that wants to try that fight: All your bullets as soon as possible, so you don't get the little guys in the way and waste a bullet, and before your aim goes sightseeing. If you can just get the worms down to three or four fat ones you should be okay even if you are out of bullets. Watch out for the ones that appear off screen though, some appear pretty late.
